Registered Nurse – Home Health

Pay Rate: $48–$52 per hour
Shift: 8:00 a.m. – 4:30 p.m. (On-call rotation required at least every 3 weeks)
Coverage Area: Yazoo and Holmes County

Position Overview

We are seeking an experienced and compassionate Registered Nurse (RN) to join our home health team. The RN will provide skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care that promotes independence and optimal outcomes.

Primary Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.

• Collaborate with physicians, social workers, and other healthcare professionals to ensure coordinated and effective care delivery.

• Provide wound care, IV therapy, Foley catheter management, and patient/family education.

• Complete OASIS documentation accurately and timely.

• Identify and address barriers to care, ensuring cost-effective and appropriate service levels.

• Facilitate care transitions and coordinate necessary community and medical resources.

• Promote health education and preventive care strategies.

• Maintain detailed clinical records in compliance with organizational and regulatory standards.

• Serve as a patient advocate, ensuring privacy and confidentiality at all times.

Qualifications

• Required: Active Mississippi RN license or Compact License in good standing.

• Required: CPR Certification.

• Experience: Minimum of one year of home health experience with proficiency in HCHB (Homecare Homebase) preferred.

• Proficiency in wound care, OASIS documentation, IV therapy, Foley care, and patient teaching.

• Strong communication, organizational, and critical-thinking skills.

• Associate’s or Bachelor’s degree in Nursing (BSN preferred).

• Case Management Certification preferred.
