Registered Nurse (RN) – Hospice Care
Schedule: Monday–Friday, 8:00 am – 4:30 pm
Shift Type: Days | No weekends | No on-call required
Overview
We are seeking experienced and compassionate Registered Nurses (RNs) to join our hospice care team. The RN will provide high-quality clinical care to patients receiving end-of-life services, ensuring dignity, comfort, and support for both patients and their families.
Candidates must be comfortable working independently, managing a daily caseload, and delivering comprehensive nursing assessments and interventions across a variety of care settings.
Responsibilities

Perform comprehensive nursing assessments including admissions, recertifications, routine, PRN, and death visits.

Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.

Ensure quality and compliance through accurate and timely documentation in the EMR system (HomeCare Homebase experience strongly preferred).

Maintain productivity benchmarks of 5–7 visits per day while delivering compassionate, patient-centered care.

Coordinate with physicians, social workers, and other healthcare professionals to facilitate care transitions and appropriate levels of service.

Identify and address social determinants of health that impact patient care (e.g., housing, transportation, or community resources).

Promote health education, preventive care, and emotional support for patients and families.

Advocate for patient rights while maintaining confidentiality and ethical standards.

Participate in interdisciplinary team meetings and contribute to continuous quality improvement initiatives.
Qualifications

Active RN license in good standing (state-specific).

Current BLS/CPR certification required.

Valid driver’s license and auto insurance.

Hospice experience required; HomeCare Homebase (HCHB) proficiency strongly preferred.

Associate’s or Bachelor’s degree in Nursing (BSN preferred).

Strong clinical judgment and critical-thinking skills in complex situations.

Exceptional communication and organizational abilities.

Case Management certification preferred, but not required.
