Registered Nurse – Care Manager
Pay Rate: $55–$60/hr
Location: Fresno, CA
Schedule: Hybrid – Monday through Thursday in the field, Friday remote (if applicable)
Mileage Reimbursement Provided
Overview
We are seeking an experienced and compassionate Registered Nurse (RN) to join our care management team. The Care Manager will work collaboratively as part of an integrated care team that includes advanced practice providers and social workers. This role focuses on supporting patients with complex and chronic health conditions, improving their quality of life at home, and helping to slow disease progression through proactive and coordinated care.
Key Responsibilities

Partner with patients’ healthcare providers to develop, implement, and continually adapt individualized care plans.

Perform in-home care management visits to assess patient needs and execute care plans.

Monitor biometric data and follow clinical protocols for any required interventions.

Reconcile medications and coordinate with pharmacists and prescribers as needed.

Conduct patient assessments, health surveys, and education sessions on chronic disease management (e.g., CKD, ESRD, dialysis).

Promote adherence to treatment and medication plans through consistent communication and support.

Engage family members and social supports in the care process to enhance patient outcomes.

Serve as the primary point of contact for patients during business hours.

Educate patients on medications, medical conditions, diet, exercise, and healthy lifestyle choices.

Support patients in making proactive care decisions, including advance care planning.

Conduct hospital discharge and post-operative visits to ensure smooth transitions of care.

Document all care updates and patient progress accurately in care management systems.

Coordinate with dialysis and specialty providers to ensure continuity of care.
Qualifications

Active Registered Nurse (RN) license in the state of California.

Graduate of an accredited school of nursing.

Minimum 2 years of experience in care management or working with patients managing chronic conditions such as CKD or ESRD.

Basic Life Support (BLS) certification (support available for completion upon hire).

Valid driver’s license and reliable transportation for local travel.

Ability to work independently with minimal supervision.

Strong empathy, communication, and relationship-building skills.

Proficient in Microsoft Office and mobile/web-based applications.

Ability to participate in occasional after-hours on-call support and limited travel for training.
