Registered Nurse (Hospice/Home Health/ Case Manager) – On-Call Weekend Coverage
Location: Grand Junction, CO 81506
Coverage Area: Mesa and Garfield Counties
Pay Rate: $48–$52 per hour
Schedule: On Call from Friday at 5pm to Monday at 8am
Shift Requirements

Schedule: On-call weekend position starting Friday at 5:00 PM through Monday at 8:00 AM.

Travel: Required within Mesa and Garfield Counties.

Preferred Experience: Hospice care and familiarity with HCHB (HomeCare HomeBase) or PointCare systems.
Position Summary
We are seeking a compassionate and skilled Registered Nurse (RN) to provide on-call hospice care services. This role is responsible for performing comprehensive clinical assessments, developing individualized care plans, and coordinating with interdisciplinary teams to ensure quality, patient-centered outcomes.
Primary Responsibilities

Conduct thorough assessments of high-risk patients and identify appropriate care interventions.

Collaborate with physicians, social workers, and care teams to develop and implement individualized care plans.

Monitor patient progress and reassess care plans to ensure efficient and appropriate levels of service.

Facilitate coordination of care transitions, including discharge planning and follow-up.

Document clinical, functional, and financial outcomes in accordance with organizational standards.

Engage specialty and community resources to support patient care needs.

Educate patients and families on treatment options, disease management, and wellness strategies.

Identify opportunities for health promotion and illness prevention.

Serve as a patient advocate while maintaining the highest standards of privacy and confidentiality.

Respond promptly to urgent or emergent patient needs during on-call hours.
Qualifications

Education: Associate’s Degree in Nursing (ADN) required; BSN preferred.

Licensure: Active Colorado RN license or valid multi-state compact license.

Experience: Previous hospice or home health experience strongly preferred.

Skills: Strong clinical judgment, critical thinking, and communication abilities.

Requirements: Reliable transportation and current auto insurance.

Technical Skills: Familiarity with HCHB or PointCare systems preferred.
