Home Health Physical Therapist
Position Summary:
Provide expert physical therapy care in the home setting, helping patients regain mobility, independence, and confidence. Deliver personalized treatment plans, educate patients and caregivers, and collaborate with the care team to achieve optimal outcomes.
Key Responsibilities:

Conduct patient assessments and develop individualized treatment plans

Implement therapy programs to improve strength, balance, and function

Educate patients and caregivers on exercises and activity modifications

Accurately document visits using HCHB and OASIS systems

Communicate progress and coordinate care with the interdisciplinary team
Qualifications:

Bachelor’s or Master’s degree in Physical Therapy from an accredited program

Active, unrestricted state license to practice Physical Therapy
-
Proficiency with Homecare Homebase (HCHB) and OASIS documentation
-
Strong communication, organizational, and patient care skills
-
Experience in home health preferred
