Registered Nurse (RN) – Home Health
Pay Rate: $55–$59/hr
Mileage Reimbursement: $0.45 per mile
Schedule: Monday–Friday (Flexible)
On-Call: 1 week per month (easy rotation)
Coverage Area: Charlottesville to Staunton, VA
Position Overview
We are seeking an experienced Registered Nurse (RN) to provide high-quality, patient-centered home health services. The ideal candidate will be comfortable managing a flexible schedule, completing required daily productivity points, and collaborating with an interdisciplinary team to ensure exceptional outcomes and continuity of care.
Primary Responsibilities

Conduct comprehensive assessments for high-risk members and patients in their home setting.

Collaborate with primary care providers and other healthcare professionals to develop and implement individualized, goal-oriented care plans.

Identify and coordinate cost-effective, appropriate levels of care across the continuum.

Facilitate non-medical supports (housing, transportation, etc.) to promote adherence to care plans.

Manage and coordinate care transitions to ensure seamless continuity between providers, facilities, and home settings.

Partner with physicians, social workers, discharge planners, and claims professionals to support optimal patient outcomes.

Maintain detailed documentation of clinical, functional, and financial outcomes.

Identify opportunities for health promotion, illness prevention, and patient education.

Act as a patient advocate while maintaining strict confidentiality and HIPAA compliance.

Support the prevention and early intervention of adverse patient outcomes.
Qualifications

Education: Associate’s Degree in Nursing (BSN preferred).

Licensure: Current, unrestricted multi-state RN license (Compact).

Experience: Minimum of 1–2 years of clinical experience; home health experience preferred.

Certifications:

Current CPR/BLS certification

TB test clearance

Wound care/competency test completion

Case Management Certification preferred

Technical Skills: Proficiency with OASIS documentation and Homecare Homebase (HCHB) required.

Strong analytical and decision-making skills in assessing complex medical situations.

Excellent communication, time management, and patient engagement abilities.
