Job Summary
The Licensed Practical Nurse (LPN) provides direct patient care in accordance with individualized plans of care, physician orders, and organizational policies. The LPN promotes patient safety, dignity, and independence while delivering high-quality nursing services in a home health or clinical setting.
Key Responsibilities

Provide patient care in accordance with established plans of care and physician instructions

Monitor and document vital signs, reporting abnormal findings promptly

Administer medications, treatments, feedings, oxygen therapy, and ostomy care as prescribed

Perform wound care, drain care, and G-tube management as required

Educate patients and caregivers on basic care techniques and disease processes

Collaborate with interdisciplinary team members including Physicians, Physical Therapists, Occupational Therapists, and Speech Therapists

Maintain compliance with infection control policies and procedures
-
Identify and report potential communicable diseases or infections
-
Accurately document all nursing care, patient responses, progress toward goals, and communications
-
Order and maintain adequate medical supplies, medications, and nutritional support
-
Monitor and assess patient incidents; complete incident reports accurately and in a timely manner
-
Ensure medical equipment is properly maintained and report malfunctioning equipment to the appropriate DME provider
-
Support patient safety, comfort, and independence at all times
