Location: Greater Danbury Area, CT (Danbury, Newtown, New Milford, New Fairfield, Brookfield, Roxbury, Redding, Ridgefield, Bridgewater, and Bethel)
Assignment Length: 3 Months
Schedule: Monday – Friday | 8:30 AM – 5:00 PM
Start Location: Middlebury, CT
Start Time: 8:30 AM
Dress Code: Scrubs
Position Overview:
We are seeking an experienced Home Health RN to provide interim coverage for a vacated territory in the Greater Danbury Area and surrounding counties. The ideal candidate will have a strong background in home care nursing and case management, with proficiency in HCHB (Homecare Homebase) software.
Territory Coverage:

Fairfield County: Danbury, Newtown, New Milford, New Fairfield, Brookfield, Redding, Ridgefield, Bridgewater, and Bethel

Litchfield County: Roxbury
Responsibilities:

Provide comprehensive case management and skilled nursing care to home health patients

Coordinate care plans and communicate with interdisciplinary teams

Conduct patient assessments and monitor clinical progress

Ensure timely documentation using HCHB
Requirements:

Active RN license in Connecticut
-
Minimum 1 year of home care experience

Proficiency in HCHB (Homecare Homebase)

Strong communication and time management skills
