Case Manager RN – Intake Calls (Remote with Local Travel)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Contract: 20+ weeks with potential to extend or transition to a follow-up RN role
Pay: $40-50/hr

Our Client is a rapidly growing healthcare organization specializing in Chronic Kidney Disease (CKD) and End-Stage Renal Failure (ESRF). Partnering with leading insurance companies, they deliver value-based care—focusing on keeping patients healthy and reducing unnecessary hospital admissions. Due to exceptional patient outcomes, they are expanding into new states and serving more members than ever before.
Position Overview:
We are seeking experienced Case Manger/ Homea Health/ Hospice/ Nephrology Registered Nurses to join the Intake team. In this role, you will conduct comprehensive intake assessments for new members referred by insurance partners. These assessments are critical in ensuring members receive the right care from the start.
Key Responsibilities:
- Conduct 30–90 minute intake calls with 4–6 new members daily to assess their health status and chronic conditions.
- Document findings in Salesforce (primary platform) and Athena EMR.
- Collaborate with providers (NPs and MDs) to transition members into the appropriate ongoing care pathway.
- Provide occasional in-person visits within a one-hour commute for members requiring in-home assessments based on acuity.
- Maintain accurate records and communicate effectively with the care team.
Qualifications:
- Active RN license in practicing state (Illinois)
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum 3 years of nursing experience.
- Background in Home Health, Palliative Care, Hospice, Dialysis, Nephrology, or other chronic disease specialties (CKD experience strongly preferred).
- Strong computer skills; Salesforce experience is a plus (training provided if needed).
- Valid driver’s license, reliable transportation, and the ability to travel locally as required.
- Reliable high-speed internet and phone connection for remote work.
