Position Overview
The Registered Nurse (RN) Case Manager provides comprehensive assessments and care coordination for high-risk members in a home health setting. The RN collaborates with providers and interdisciplinary teams to create individualized care plans, ensuring appropriate levels of care and optimal patient outcomes.
Key Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans.

Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care.

Facilitate care transitions and ensure continuity of care post-discharge.

Monitor progress and adjust care plans as needed for optimal outcomes.

Identify barriers to care and implement non-medical supports (housing, transportation, etc.).

Maintain accurate documentation of clinical and financial outcomes.

Promote preventive care and wellness education for patients and families.
-
Advocate for patient rights, privacy, and confidentiality.
Qualifications
-
Associate’s Degree in Nursing (BSN preferred)

Active, unrestricted Registered Nurse license

Strong ability to interpret complex clinical data and make informed decisions

Prior Case Management experience preferred

Homecare Homebase and OASIS proficiency required
