Physical Therapist – Home Health
Schedule: Full-Time, Monday–Friday, 8-hour shifts
Hours: 40 hours per week
Workload: Approximately 30 units per week
Location: Baltimore, MD area (city and surrounding counties)
Position Overview
We are seeking a licensed Physical Therapist to join a home health team providing patient-centered care in the community. The ideal candidate will have experience in home health and strong clinical skills, with a passion for helping patients maximize independence and quality of life.
Responsibilities

Perform physical therapy evaluations and treatments based on physician referrals.

Complete comprehensive assessments using appropriate tools and procedures.

Review patient records and histories to ensure safe and effective care planning.

Establish measurable goals and individualized treatment plans to support patient independence and function.

Reassess progress regularly and adjust treatment plans as needed.

Collaborate with physicians, care teams, and family members to ensure coordinated care.

Educate patients and caregivers on exercises, functional mobility, and use of adaptive equipment.

Document all evaluations, treatments, and progress in accordance with regulatory and organizational standards.

Supervise Physical Therapist Assistants and Aides as required by state practice guidelines.

Participate in staff development, team meetings, and continuing education opportunities.
Requirements
-
Current, active Physical Therapist license in the state of Maryland.

Prior home health experience required.

Knowledge of OASIS documentation required.

Valid driver’s license and reliable transportation.
Preferred Qualifications
-
Experience with HCHB (Home Care Home Base) or similar electronic medical record systems.

Strong communication skills and ability to work independently in the field.
-
Experience supporting diverse patient populations, including elderly and medically fragile individuals.
