Licensed Practical Nurse (LPN) – Home Health
Location: Kentucky (Compact License Accepted)
Shift: Monday–Friday, 8:00 AM – 5:00 PM
On-Call: 1 weekday and 1 Saturday per month
Contract Type: Contract
Worker Type: Onsite
Pay: $30-35/r
Overview
The Licensed Practical Nurse (LPN) is responsible for delivering quality patient care in a home health setting in accordance with the client’s plan of care. The LPN will monitor and record patient health data, administer medications, and ensure that infection control procedures are followed at all times. This position requires strong attention to detail, excellent communication skills, and a compassionate approach to patient care.
Responsibilities

Provide nursing care as outlined in the client’s individualized plan of care while maintaining dignity, safety, and independence.

Monitor and record vital signs, promptly reporting abnormal findings to the supervising RN or physician.

Administer medications, enteral feedings, and oxygen therapy per orders and document results appropriately.

Perform wound care and IV therapy when indicated and within scope of practice.

Collaborate with interdisciplinary care team members (PT, OT, Speech Therapy, etc.) to ensure continuity of care.

Maintain strict adherence to infection control policies and report any suspected communicable diseases.

Accurately document all nursing actions, communications, and patient progress toward goals.

Ensure adequate inventory of medical supplies, medications, and nutrition items as needed.

Inspect, maintain, and coordinate repair or replacement of medical equipment as required.

Monitor and report all incidents or unusual occurrences, ensuring timely and accurate completion of incident reports.
Qualifications
-
License: Active LPN license in Kentucky or valid Compact State license.

Experience: Prior home health experience strongly preferred.

Skills:
-
HCHB (Homecare Homebase) and PointCare software proficiency preferred.

Wound care and IV therapy experience desirable.
Other: Strong clinical judgment, effective communication, and commitment to patient-centered care.
